This remote nursing role focuses on helping members achieve and maintain their best possible health outcomes through personalized, telephonic care management. You will assess individual needs, identify barriers to effective care, and connect members and families with appropriate clinical, community, and social resources. The position combines clinical expertise, critical thinking, health coaching, and care coordination in a member-focused environment. You will continuously monitor progress, develop and adjust care plans, and help guide members toward meaningful health outcomes. As a bilingual professional, you will communicate confidently in both English and Spanish while supporting members with diverse needs.
