Our Clinical Wound Care & Home Health team combines technology and clinical judgment to provide coordinated, whole-person care that helps members heal and remain safely at home. As a Wound Care Certified Nurse Practitioner, you will oversee care for members with complex and chronic wounds from initial assessment through closure or transition of care. You will review high-definition wound images and clinical data through a secure, AI-enabled platform, independently apply clinical judgment, and address broader health and social needs affecting healing.
