Social Worker

This fully remote role provides behavioral health and social support to members facing complex health and social needs.You’ll assess psychosocial circumstances, identify social determinants of health, and help remove barriers to care.The role combines telephonic outreach, supportive counseling, care coordination, and community resource navigation.You’ll work closely with an interdisciplinary clinical care team to support comprehensive member outcomes.The position offers an opportunity to make a meaningful difference by connecting individuals and families with practical resources and services.Success requires strong communication, cultural awareness, organization, and the ability to work independently.Limited travel may be required based on business needs.
