Senior Nurse Case Manager – Rare Disease

This role offers an opportunity to make a meaningful impact on patients navigating complex rare disease treatments and support services. You will manage patient cases across an assigned geography while partnering with physicians, hospitals, clinics, insurers, pharmacies, and other healthcare stakeholders. Your work will help coordinate reimbursement, medication access, clinical monitoring, adherence, and resolution of treatment-related challenges. You will combine clinical expertise with strong communication and case-management skills to support both patients and healthcare professionals. The position also involves contributing to disease-management programs, training, and continuous improvement initiatives. It is well suited to an experienced bilingual nursing professional who is compassionate, organized, and comfortable working independently.
