Position Summary
Support the Care Management Center of Excellence (CM COE) with operationalizing the care management components of program and system implementations. The Implementation Lead is accountable for the implementation and ongoing management of CM COE projects. This role has primary responsibility for successfully leading assigned OneCM migrations, ensuring operational readiness, stakeholder alignment, and sustainable adoption of care management processes and systems.
Fundamental Components

Engage stakeholders and align cross‑functional teams on new delivery models and processes
Facilitates alignment sessions, clarifies impacts, manages expectations, and ensures organizational understanding of upcoming changes.

Lead business readiness and change management activities
Develops and executes communication plans, readiness assessments, role impact analyses, and operational transition activities.

Design and coordinate pilot launches and phased adoption waves
Establishes pilot objectives, measures success, gathers feedback, and iterates before broader rollout.

Oversee performance measurement, adoption tracking, and benefits realization
Monitors key metrics, identifies adoption barriers, and ensures the program delivers expected operational outcomes.

Support issue resolution, continuous improvement, and stabilization after go‑live
Partners with teams to resolve post‑launch challenges and embed new ways of working into standard operations.
Required Qualifications
- At least 4 years of demonstrated success leading large-scale initiatives and projects.
- Ability to navigate and manage multiple systems and databases, along with strong proficiency in the MS Office Suite.
- Strong written and verbal communication skills, with the ability to engage effectively with stakeholders at all organizational levels—from C‑Suite leaders to frontline staff. Ability to build, maintain, and support productive relationships across cross-functional teams is key to success.
- Proven ability to collaborate with internal and external partners to expand knowledge and improve our implementation processes.
- Skilled in group facilitation and presentations that inform, influence, and drive action across diverse audiences.
- Strong knowledge and proven experience in project management principles and practices.
Preferred Qualifications
- Demonstrated ability to influence people and optimize implementations without direct authority.
- Comprehensive understanding of managed care case management and care coordination, including CM systems and documentation standards.
- Comprehensive understanding of business readiness practices particularly with system migrations.
- Skilled in managing multiple project timelines simultaneously and consistently providing clear, evidence‑based progress updates across all phases of implementation.
- Actively contribute to strategic initiatives and process‑improvement efforts by applying lessons learned.
- Proficient in Microsoft Project, Asana, Monday.com and other project-management tools.
- Adept at managing client expectations and fostering confidence through clear communication and reliable delivery.
Education
- Bachelor’s degree preferred, or equivalent experience considered

Pay Range
The typical pay range for this role is:
$75,400.00 - $182,549.00
This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above. This position also includes an award target in the company’s equity award program.
