Position Summary

The Senior Epic Analyst, Healthy Planet & Care Coordination leads the design, build, and optimization of Epic solutions that support population health management, care management workflows, and coordinated care delivery. This role focuses on Healthy Planet (registries, care plans, care teams, health maintenance) alongside Care Coordination/Care Everywhere workflows that support transitions of care, referral management, and cross-continuum communication. The Senior Analyst acts as a subject matter expert, mentoring junior analysts while partnering closely with care management, population health, and clinical leadership to translate program strategy into scalable Epic solutions.

Key Responsibilities

Healthy Planet Build & Optimization

  • Design, build, and maintain Healthy Planet registries, care plans, care teams, and health maintenance topics to support population health and value-based care initiatives.
  • Configure and optimize risk stratification tools, care gap identification, and outreach workflows within Healthy Planet.
  • Lead testing and validation of Healthy Planet builds to ensure accuracy of registry logic, care plan triggers, and reporting outputs.

Care Coordination Workflow Design

  • Design and maintain care coordination workflows supporting transitions of care, discharge planning, referral management, and care team communication.
  • Optimize Care Everywhere and interoperability workflows to support timely, accurate information exchange across care settings.
  • Partner with care management and case management teams to streamline documentation, task management, and communication tools within Epic.

Stakeholder Partnership & Requirements Gathering

  • Serve as the primary Epic liaison for Population Health, Care Management, and ACO stakeholders, translating program goals into technical requirements.
  • Facilitate workflow design sessions with clinical and operational leadership to identify pain points and optimization opportunities.
  • Present build recommendations, timelines, and trade-offs to leadership and governance committees.

Mentorship & Team Support

  • Mentor junior and mid-level analysts on Healthy Planet and care coordination build standards, testing practices, and troubleshooting techniques.
  • Review and validate build work completed by other team members prior to release.
  • Contribute to team documentation, standard operating procedures, and onboarding materials.

Reporting, Governance & Release Management

  • Partner with reporting teams to ensure Healthy Planet and care coordination data feeds support accurate dashboards and quality/outcomes reporting (e.g., Reporting Workbench, Clarity/Caboodle, SlicerDicer).
  • Participate in change control, release planning, and regression testing for Healthy Planet and care coordination builds.
  • Maintain up-to-date documentation of workflows, build configurations, and design decisions for audit and knowledge-continuity purposes.
  • Manage and groom user stories/backlog items in agile tools (e.g., Rally) to support accurate leadership reporting and release planning.

Required Qualifications

  • Bachelor's degree in Health Informatics, Information Systems, Nursing, or related field (or equivalent experience).
  • 4+ years of Epic experience, with substantial hands-on work in Healthy Planet and/or care coordination/care management modules.
  • Epic certification(s) in Healthy Planet, Care Everywhere, Ambulatory or related modules required or strongly preferred.
  • Demonstrated experience designing workflows for population health, care management, or value-based care programs.
  • Strong understanding of transitions-of-care processes and cross-continuum care delivery.

Preferred Qualifications

  • Experience in an ACO, health system, or integrated delivery network with population health responsibilities.
  • Familiarity with FHIR/HL7 interoperability standards and Care Everywhere configuration.
  • Experience mentoring or informally leading junior analysts.
  • Familiarity with agile delivery tools (e.g., Rally) for backlog and release management.

Pay Range

The typical pay range for this role is:

$83,430.00 - $222,480.00

This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
