This role provides comprehensive case management for members with complex health needs, using advanced clinical knowledge to support high-quality and cost-effective care.You will assess health status, identify needs and resources, and develop individualized care plans in collaboration with members and healthcare providers.The position combines clinical assessment, care coordination, advocacy, monitoring, and evaluation to help members achieve optimal health outcomes.You will serve as a trusted liaison across physicians, providers, and care teams while ensuring members have access to appropriate services and resources.The role also contributes to responsible healthcare utilization by identifying, resolving, and preventing inappropriate use of member benefits.As a senior-level professional, you will provide clinical expertise, mentorship, and problem-solving support to other Case Managers.The position is primarily remote in Pennsylvania, with occasional physical-site requirements and less than 25% travel.
