This is a fully remote senior care management opportunity focused on coordinating complex patient cases and supporting continuity of care.You’ll help patients navigate clinical, social, and access-related barriers throughout their treatment journey.The role involves close collaboration with healthcare providers, pharmacies, payers, and internal teams.You’ll assess cases, support care plans, facilitate transitions, and help resolve issues that may affect patient outcomes.You’ll also analyze recurring case and workflow challenges and contribute to practical process improvements.As a senior team member, you’ll provide guidance on complex cases, escalations, documentation, and best practices.The position offers a collaborative environment where clinical expertise, communication, organization, and patient focus are highly valued.
