Clinical Services Coordinator

As a Clinical Services Coordinator, you will play a central role in connecting patients with the care, services, and resources they need.You will support patient outreach, scheduling, care coordination, preventive care, and transitions of care across diverse populations.Working closely with interdisciplinary teams, you will help improve access, close care gaps, and strengthen continuity of care.You will use electronic medical records, population health data, AI-enabled tools, and reporting platforms to identify opportunities for intervention.The role also contributes to quality improvement, workflow optimization, and the development of technology-enabled clinical services.You will work with patients, caregivers, providers, community organizations, and care teams in a collaborative, mission-driven environment.This fully remote opportunity is ideal for an experienced care coordination professional who is bilingual in English and Spanish and passionate about improving health outcomes.
