Our partner is looking for a Case Management Coordinator - Transition of Care Team based in the United States. This full-time remote role supports members as they navigate healthcare services, benefits, appointments, and transitions in care.You will use critical thinking, care-management resources, and a member-centered approach to identify needs and remove barriers to appropriate care.The role combines case coordination, benefit navigation, health education, documentation, and collaboration with clinical and multidisciplinary teams.You will help identify higher-risk situations and ensure members receive appropriate referrals, services, and follow-up.Success requires strong communication, sound judgment, organization, and the ability to motivate members toward informed healthcare decisions.You will work within established quality, regulatory, accreditation, and privacy standards while managing a high volume of member needs.This is a structured remote environment with a standard 8:00 AM–5:00 PM schedule in the assigned market time zone.
