This role serves as a dedicated subject matter expert on Veteran-specific healthcare, benefits, and community resources.You will help care teams navigate VA systems and connect Veterans with the appropriate services and support.The position combines care coordination, benefits education, workflow development, training, and direct member support.You will provide guidance on complex cases while helping establish effective processes, escalation pathways, and program standards.The role also supports care coordinators through education on VA resources, social determinants of health, and common barriers Veterans may face.Success requires strong clinical judgment, empathy, communication, and the ability to build trust with diverse populations.This is a fully remote, non-supervisory position offering the opportunity to make a meaningful impact on Veteran health and well-being.
